  “I am writing to express my profound gratitude for the exceptional care provided by your team during an incredibly difficult chapter for my family. ​Beyond the beautiful, serene environment of the facility, which provided a necessary sense of peace, it was the clinical and human standard of care that truly made the difference. I was deeply impressed by the transparency regarding the hard, necessary facts of our situation; your team’s ability to communicate difficult realities with clarity and honesty was invaluable and provided me with much-needed closure. ​Furthermore, the compassionate, dignified handling of my loved one throughout our stay ensured they we were treated with the utmost respect, which brought us immense comfort. I also want to specifically commend your staff for the emotional and mental support provided to my entire party. Your ability to hold space for our grief while remaining steadfast in your duties was remarkable. ​While the emotional weight of these circumstances makes it difficult to recall every name, I would like to extend my sincere appreciation to the entire staff, with special thanks to Robin, Cat, Catherine, Megan, Fredricka, Alexander, Eddie, Annie, and Dr. Bob & Deese.   “My Mother was under the care of Transitions Hospice Care beginning in December 2025 at the nursing facility where she lived. She sadly passed away last week. I cannot say enough positive things about the support both she and I received from Transitions. As my Mother’s only close person, their care was truly a lifeline for me. Having regular visits, updates, guidance, and simply an extra set of expert eyes brought enormous reassurance during such a difficult time. A very sincere thank you to Rahma Azzan, her wonderful hospice nurse, and Lauren, the social worker on the team, whose kindness and support meant so much to me. I always knew I could call Rahma, Lauren, or Transitions at any time if I had questions or needed help. I am deeply grateful for the compassionate care they provided to my Mother and to me. If your loved one needs hospice care, I would not hesitate to recommend Transitions.”
